Sports Physicals
There will be a sports physical night sponsored by the PHS Booster Club on Wednesday, August 7th at Philomath Family Medicine. Sports physicals will be given from 6-7pm. Physicals are given on a first come, first served basis and the cost is $25 (you will not be able to bill your insurance). Checks may be made out to the Philomath Booster Club.
Volleyball Camp
For her PHS Senior Project, Courtney Croy will be putting on a volleyball camp this summer for middle school students. The camp is scheduled for July 19-20, 2013. Additional information and registration is attached.

Sports Physicals
All student that wish to participate in sports at Philomath Middle School will need to have a doctor's physical on file in the school office. If you are new to sports at the middle school or need to update your physical, pick up the paperwork in the office or download the following link to take with you to the doctor. The form is also available on our PHS Athletics website or at osaa.org under forms. This physical is valid for 2 years from the date your physician signs the form.

Students who have a physical on file will only need to submit the EMERGENCY MEDICAL/ANNUAL MEDICAL HISTORY form to the school office before their first sport begins.

SCHOOL INSURANCE FOR SPORTS
All students must have some kind of health insurance to play sports at Philomath Middle School. If your child does not have health insurance, a low cost option is available through K&K Insurance Group. The accident coverage for at school costs are $22.00 - $29.00 for one year depending on which plan you choose. 24 hour coverage is also available for $69.00 - $102.00 per year. Other options are available through the Oregon Health Plan under "Oregon Healthy Kids" see the District web site for information under the Health Services link.
